The Electronic Health Record (EHR) is the digital tool that collects all information about a citizen's health in one space.
Thanks to the EHR you can consult reports, prescriptions, exemptions, vaccinations and the visit history without having to keep paper documents.

🔹 How to activate it

1. Access with SPID, CIE or CNS to the regional portal of the Electronic Health Record.

   • Each Region has its own dedicated site (e.g. for Emilia-Romagna: fascicolo-sanitario.it).

2. Follow the online activation procedure or at a CUP/USL counter.

3. After activation, you will have immediate access to your health data.
